Pre-Procedure Intervention of Subclinical Hypothyroidism: Case Report

Background: Subclinical hypothyroidism (SH) is a very common disorder in the general population. In patients with known or who have been undergoing treatment, TSH test should be included preoperative assessment to determine adequacy of treatment and ensure that thyroid therapy optimized before surgery. Case Presentation: A 39 years old woman complains about weakness has 4 interfering activities. Shortness breath if doing strenuous activities, walking long distances, climbing stairs, shortness decreases after rest. Sore throat found since 1 week hospitalization, fever for day. lump neck not found. Previously, patient had treated 8 days Department Cardiology planned installation temporary pacemaker due bradycardia persisted until cardiac arrest. Conclusion: hypothyroid pre-procedure intervention are predisposed anemia, electrophysiologic disturbances, hypotension, all which can precipitate cardiovascular collapse. The goal perioperative dysfunction centers around attempt normalize hormone levels surgical whenever possible and, when feasible, use other measures will maximize hemodynamic stability prevent decompensation.
